We are seeking a skilled and empathetic Community Officer to join our team. In this role, you will play a pivotal part in delivering high-quality Home and Community Care (HACC) services.
Your Responsibilities:
* Managing care worker schedules through a computerised rostering system to meet client needs.
* Providing expert guidance, training, and support to Community Care Workers, including home visits when necessary.
* Monitoring service performance and ensuring compliance with Aged Care Quality Standards, HACC service standards, and occupational health and safety requirements.
* Collating and responding to feedback from clients and carers, driving continuous service improvements.
* Supporting programs like community meals and senior citizen group activities.
* Participating in on-call rosters to provide around-the-clock support to clients and carers.
* Undertaking key administrative tasks, including training records, newsletters, and resource management.
Requirements:
* Minimum Certificate IV in Community Services or extensive HACC experience.
* Knowledge of person-centred care and strengthened aged care quality standards.
* Strong communication, negotiation, and consultative skills.
* Ability to work independently, manage priorities, and exercise initiative.
* Current First Aid/CPR certification, driver's licence, and a satisfactory police record check.
