About the Role
At Lutheran Care, we believe the way we support people shapes the impact we have in the community. We're looking for compassionate Pathways Workers who thrive on making meaningful connections and helping individuals and families access safe, respectful, and culturally responsive support.
As a Pathways Worker, you'll be on the frontline, meeting people where they are, listening to their stories and providing short‑term support through risk assessment, brief interventions and coordinated referrals.
You'll draw on sector frameworks, structured assessment tools and the referral database to complete clear, consistent and trauma‑informed assessments. Working closely with your Team Leader and partner services (Yilli Rreung Aboriginal Corporation, The Salvation Army), you'll ensure people are connected quickly, safely and appropriately to the supports that best meet their needs.
Key Responsibilities:
* Be the first point of contact for people seeking support, through phone and in-person services.
* Conduct brief, trauma‑informed intake assessments and support clients to understand their options.
* Provide short‑term support only, including brief interventions and warm referrals to partner services for ongoing support.
* Document all client interactions clearly and within required timeframes.
* Respond to disclosures and complex risk presentations using calm, trauma‑informed communication and de‑escalation techniques
* Work collaboratively with partner organisations to ensure clients receive coordinated, culturally responsive support.
