* Primary degree
* Fellowship
* CPD certificate (if applicable)
* References
* CRC
* NSW WWCC
* Indemnity
* Medicare provider number

* $1,750 for half-day shifts
* $3,500 for full-day shifts
* $9,000 for weekend on-call with Friday sessions or $6,000 for on-call only
