Job Description:
As a Transition Care Coordinator, you will play a vital role in supporting young people aged 14–25 with chronic illness or disabilities during their transition from paediatric to adult health care systems.
You will be part of a close-knit, collaborative team that values open communication, mutual respect, and a shared commitment to supporting young people and their families.
The service assists in improving long-term engagement and outcomes by providing vital coordination support.
You'll attend paediatric and adult health clinics, support the development of self-management skills, and follow up post-transition to ensure continued engagement in care.
Key Responsibilities:
* Coordinate individualised care plans for young people with chronic illness or disabilities.
* Work directly with young people, families, and healthcare providers to ensure seamless care transitions.
* Attend paediatric and adult health clinics to provide support and guidance.
* Develop and implement strategies to improve long-term engagement and outcomes.
Requirements:
* Relevant qualifications in Nursing, Social Work, Occupational Therapy, Speech Pathology, Physiotherapy, or Dietetics.
* At least 3 years of clinical experience in the profession (Allied Health) or in the clinical area of the specified post-graduate qualification.
* Current NSW Driver's Licence and willingness to travel within the Local Health District.
