**About the role**:
Reporting to the Senior Welfare/Placement Officer this role, some of your duties for the role will include:
- Working across FMC & NHS, including some weekend work
- Referral management, including conducting placement meetings with patients and families as they transition to Residential Aged Care Facilities from hospital.
- Assisting patients and families with information about practical assistance such as access to transport and accommodation.
- Supporting patient with practical welfare support where appropriate.
- Contributing to coordination, evaluation and implementation of service improvement programs and departmental planning
- Administrative tasks directly related to a patient intervention or episode of care (as distinct from administration specific roles)
**About you**:
- Knowledge of the Aged Care Sector very desirable
- Ability to relate to and empathise with a wide range of people, including patients, families & carers, clerical staff, clinical staff, Aged Care Assessment Teams, Directors, Supervisors and Managers of Residential Aged Care Facilities and Supported Residential Accommodation facilities
- Demonstrated ability to exercise adaptability, flexibility and advocacy skills when dealing with patients, families & carers, working with distressed, confused, abusive and sometimes violent patients and families
- Experience in using a range of computer software packages, e.g. spreadsheets, word processing and database packages
