Delivering Patient-Centered Care in the Community
We invite you to join one of our Integrated Neighbourhood Teams where you will play a vital role in delivering holistic and complex nursing care within patients' homes.
This is an exciting opportunity to work with diverse caseloads led by a band 7 clinical lead and three full-time band 6 senior nurses. Our service is delivered between 8am and 6pm, seven days a week.
Our teams are co-located with community nursing, therapy, and social care colleagues, as well as out-of-hours nursing and therapy teams. We also have access to postgraduate education teams and legacy nurse support to help develop your community-specific clinical skills.
The Integrated Neighbourhood Teams cover both rural and urban areas, providing patient-centered care and working alongside Community and District Nurses, Generic Support Workers, Physiotherapists, Occupational Therapists, social workers, independence wellbeing practitioners, social prescribers, Matrons, care coordinators, and administration colleagues.
