About the Role
We are currently seeking compassionate and reliable Support Workers to join our team in Ballarat and surrounding areas on a casual basis.
At Gentle Grid, support workers are not simply carers — they are an essential part of a clinically governed, nurse-led team. You will work alongside Registered Nurses and coordinators to deliver support that is both evidence-informed and deeply human. You will provide person-centred support to NDIS participants in their homes and in the community, helping them achieve their goals, build independence, and live the life they choose.
This is a role for someone who takes pride in their work, upholds the dignity of the people they support, and understands that the little things — showing up on time, listening carefully, following a plan with precision — are actually the big things.
Key Responsibilities
- Provide personal care, including showering, grooming, dressing, and continence support
- Assist with meal preparation and domestic tasks
- Support participants to engage in community activities, social outings, and appointments
- Assist with medication as directed and within your scope
- Implement and follow individual support plans
- Build genuine, respectful, and trusting relationships with participants
- Complete accurate and timely progress notes and incident reports
-Work collaboratively with the wider Gentle Grid team and participant families/guardians
What We Are Looking For Essential:-
-A genuine passion for supporting people with disability
- Current NDIS Worker Screening Check (or willingness to obtain)
- National Police Check (within 3 months)
- First Aid and CPR Certificate (HLTAID011 and HLTAID009)
- Valid Australian driver's licence and reliable, comprehensively insured vehicle
- Right to work in Australia Desirable
- Certificate III or IV in Individual Support (Disability), Community Services, or equivalent- Previous experience in disability, aged care, or community services
- Familiarity with NDIS and person-centred approaches
