Support Worker

Offer description

About Us

Gentle Grid Disability and Nursing Services Pty Ltd is a nurse-led NDIS provider based in Ballarat, Victoria, specialising in high-intensity clinical care, evidence-informed behaviour support, and Specialist Disability Accommodation (SDA). We bridge the gap between complex healthcare needs and meaningful community living.

Our name reflects exactly who we are — a structured \"grid\" of clinical governance that remains \"gentle\" enough to always put the participant's voice first. Led by Registered Nurses, we ensure that even the most intricate medical and behavioural requirements are met with expert oversight that enhances — not restricts — personal autonomy.

Our Vision

To be the benchmark for nurse-led disability support, where clinical complexity is met with such precision and empathy that every participant — regardless of their support needs — lives a life defined by their own choices, not their limitations.

Our Mission

To deliver evidence-informed nursing and behavioural supports that evolve with the participant, using specialised accommodation and clinical oversight to eliminate barriers to independence.

Our Values

- Clinical Accountability: We take ownership of outcomes — not just observation.

- Individual Dignity: Every participant receives the same standard of care we would expect for our own families.

- Purposeful Autonomy: Our clinical expertise exists to expand freedom, not enforce control.

- Collaborative Excellence: We work as one unified team — nurses, support workers, families, and participants together.

- Ethical Integrity: We do the right thing, even when it is the difficult thing. If you share these values and want to be part of a team that holds itself to the highest standard, we would love to hear from you.

Note: Gentle Grid is an unregistered provider, waiting for registration number from NDIS. The Audit is passed already :)
