Responsibilities
This part-time, on-site role is based in Heidelberg Heights, VIC, and involves supporting clients and coordinating their access to disability-related services. The Disability Support Coordinator will develop support plans, liaise with clients, families, and stakeholders, and monitor the implementation of support services to ensure they align with the clients' needs and goals. Daily tasks include admin task in office, managing client files, facilitating access to services, onboarding clients and ensuring compliance with NDIS requirements. Building and maintaining strong relationships with clients and service providers is a key aspect of the role.
Qualifications
* Knowledge of the NDIS framework and experience providing disability support or care services
* Strong communication, active listening, and interpersonal skills
* Case management experience, including creating support plans and managing client files
* Proficiency in administrative tasks, including managing schedules, documentation, and compliance processes
* Problem-solving skills and the ability to work independently
* Strong time-management capabilities and organizational skills
* National Police Clearance, Working with Children Check, and current First Aid and CPR certification
* Relevant qualifications in Disability, Aged Care, Social Services, or a related field (highly regarded)
* Experience working within the disability or aged care sector (desirable)
