Job Description
We are seeking a skilled Mental Health Recovery Coach to join our team. In this rewarding role, you will work collaboratively with young people aged 16-25 who have been experiencing mental health symptoms for more than six months.
About the Role
The Youth Outreach Recovery Service (YORS) is a psychosocial support model that provides eligible young people with short to medium term outreach support in the community. As a key member of our team, you will engage participants and their families in the program, developing professional and trusting working relationships.
Key Responsibilities:
1. Conduct regular face-to-face support visits to connect with the person and monitor progress.
2. Assist participants to identify areas of need using assessment tools and develop individually tailored recovery plans.
3. Provide direct practical support to participants to achieve goals, including linkages into activities and services.
4. Support participants to maintain suitable accommodation and provide guidance on physical health and wellbeing.
5. Work collaboratively with external stakeholders to ensure effective implementation of our vision and mission.
Requirements
To be successful in this role, you will require excellent communication and interpersonal skills, as well as experience working with vulnerable populations. A qualification in social work or a related field would be highly desirable.
