About the Role
* Provide personalised support to individuals with disabilities, assisting them with daily living tasks, skill development, and social engagement.
* Promote community inclusion and collaboration with families, carers, and other professionals.
* Implement Positive Behaviour Support Plans and health protocols to ensure the well-being of our clients.
Key Responsibilities
1. Deliver respectful and person-centred support aligned with individual goals and preferences.
2. Assist with personal care, domestic duties, and skill development, promoting independence and confidence.
3. Promote community inclusion and support social engagement, encouraging participation in activities and events.
4. Collaborate with families, carers, and other professionals to develop and implement individualised support plans.
5. Monitor and report any changes or concerns regarding client well-being, implementing adjustments as needed.
Required Skills and Qualifications
* Passionate about human rights and inclusion, with a commitment to providing person-centred support.
* Willingness to work flexible shifts, including sleepovers, and be available for short notice requests.
* Comfortable with travel and being part of a dynamic team environment.
* Able to communicate effectively and work collaboratively with colleagues and clients.
