About Us
Palliative Care South East provides palliative care in the southeast metropolitan region of Melbourne. We support people with a life-limiting illness to live well and, in doing so, provide support at home to improve the quality of life of individuals, carers and families. Our specialist palliative care services are provided to people of all ages, free of charge, in the Local Government areas of Cardinia, Casey, Greater Dandenong and part of Kingston.
Role
As a nurse and clinical expert within an interdisciplinary team, this role requires advanced clinical decision-making skills, with a focus on problem identification, solution-oriented thinking, and the analysis and interpretation of clinical data. The specialist community palliative care nurse will contribute to maintaining and improving clinical standards while delivering specialist palliative care—either directly or through consultative support—for clients and their families/carers.
This position plays a critical role in complex care coordination and case management, collaborating with external stakeholders or community service providers as required. It also involves rotational duties, including access, intake, and responding to clinical escalation to ensure seamless service delivery.
Recognised as a positive role model and mentor, the specialist community palliative care nurse provides guidance to less experienced colleagues, including graduate nurses, fostering skill development and professional growth. They lead within the clinical team, supporting quality improvement initiatives and research projects, and act as a key resource for others in clinical practice.
Required Skills and Experience
- Current registration with Australian Health Practitioner Regulation Agency (APHRA)
- Demonstrated ability to provide coordinated care in partnership with key external service providers
- Computer literacy – Microsoft Office suite and clinical electronic databases
- Excellent interpersonal and communication skills
- National Criminal Record Check and Working with Children Check prior to commencing employment
- A current, Full Drivers’ licence for the State of Victoria
