What You\'ll Be Doing
The Senior Clinician Community Health will be responsible for providing support to patients with late stage chronic and complex health conditions including:
- care planning and advance care planning
- palliative care needs assessments and collaborative treatment care plans
- referrals to relevant internal and external health and community support providers
- education/support to other clinicians in supporting complex patient needs.
Selection Criteria
- Current Registration as an Occupational Therapist with AHPRA or eligibility for accreditation as an Accredited Exercise Physiologist with ESSA.
- Demonstrated extensive knowledge and experience working with consumers with chronic and complex illnesses, including advance care planning discussions.
- Experience in care planning and case management of patients in the community with chronic and complex illnesses.
- Understanding of specialist palliative care, a palliative approach and community health services.
- High level interpersonal and communication skills, including written, verbal and computer skills, and ability to work collaboratively in a multidisciplinary team.
- Proven clinical leadership capabilities with the ability to provide consultancy advice, education and support to individuals, their families, allied health and nursing staff, and other health professionals.
- Current unrestricted driver\'s licence and ability to travel as required, including long distances and overnight stays.
