About the Role:
We are seeking a skilled Clinical Care Coordinator to join our multidisciplinary team. The successful candidate will contribute high-quality evidence-based clinical care, focusing on early intervention and support of young people up to the age of 25.
The role involves addressing service gaps for the 'missing middle' by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
Key responsibilities include:
* Providing comprehensive, integrated, and person-centred care across the region
* Promoting holistic wellbeing by considering broader determinants of health
* Fostering collaboration among providers to ensure a sustainable, adaptable service responsive to community needs
This is a part-time opportunity with flexibility to work 4 days per week or a 9 day fortnight. While primarily based at Gympie, there is also the option to work from home one day per week.
Requirements:
* Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, OR AASW registration/eligibility as a Social Worker, OR Level 2 ACA registration/eligibility
* Knowledge and understanding of theory underpinning evidence-based interventions and clinical practice in mental health and alcohol and drug use
* Highly developed communication and interpersonal skills
* Ability to engage with people, advocate on their behalf, and establish effective relationships with staff, clients, families & friends and stakeholders
* Experience in electronic client/customer management systems and Microsoft Office applications
* Eligibility to apply/currently hold a Blue Card for paid employment and NDIS worker screening clearance for paid employment
