About the Role
* Champion Functional Independence: Partner with a multidisciplinary team and, when needed, clients' families/carers to assess and resolve functional and mobility challenges.
* Apply Purposeful Assessment Tools: Conduct functional assessments focused on clients' specific goals and diagnoses to guide meaningful interventions.
* Recommend Life-Enhancing Equipment: Evaluate and prescribe mobility aids and coordinate with external suppliers to secure the best-fit solutions for clients.
* Design and Deliver Targeted Therapy: Create, execute, and assess personalized restorative or maintenance exercise programs, complete with clear, written recommendations to guide ongoing care.
* Lead Prevention Education: Collaborate with the interprofessional team to educate clients and carers on fall prevention and strategies to slow functional decline.
* Drive Oedema Management in Home Care: Lead and support the Home Care team in proactively preventing and managing chronic oedema.
* Connect Clients to Reablement Supports: Inform clients and families about disability and reablement options, and facilitate referrals to further services as needed.

About You
* Qualifications in Physiotherapy
* Registered with AHPRA
* Current NSW driver license and use of registered motor vehicle
* Current NDIS Worker Check (NDISWC)
* Experience in assessment and intervention working with children and young people
* Experience in assessment and intervention working with young people and adults with intellectual disability
* Experience in the assessment, and planning individual restorative or maintenance exercise programs for older persons
* Experience in falls prevention education
