The Multidisciplinary Care Coordinator is a key position that requires effective engagement with the allied health sector in western NSW PHN region to strengthen multidisciplinary team-based care in primary care.
Key Responsibilities:
* Create strong relationships with clinicians, stakeholders, and communities
* Identify key challenges and opportunities to strengthen multidisciplinary team-based care in primary care settings
* Leverage critical enablers such as digital health, continuous quality improvement activities, education, and commissioning
This role will provide leadership on coordination, advocacy, education, and establishment of allied health partnerships with General Practices and ACCHOs to influence and support implementation of evidence-based care.
Selection Criteria:
* Experience working in primary care, such as General Practice, Aboriginal health services, allied health services, community pharmacy, or Community Health services
* Excellent interpersonal skills, with well-developed written communication skills
* Project management experience, including creating and managing project timelines
* Demonstrated understanding of Aboriginal culture, including commitment to cultural awareness and safety
