About the role We are currently seeking Community Support Workers to join our Individualised Community Living Strategy (ICLS) program.
In this role, you will provide outreach support to individuals living in the Perth metropolitan area and surrounding regions.
The service supports clients with a mental health diagnosis who are eligible for housing support.
The ideal candidate will be based South of the River and have access to a reliable vehicle with the flexibility to travel up to 40 minutes from home, as required.
Travel will be within a 40 km radius of the metro area.
Shift periods are available Monday to Saturday between 8am – 6pm, with each shift lasting 2 to 3 hours.
The role of the Community Support Worker is to help clients access their local services and maximise their recovery success while living independently in the community.
Key Responsibilities Support the consumer to increase their ability to fully participate in their ongoing clinical and psycho-social support needsAssist consumers to develop their skills to self-manage their lifestyle and well-beingDevelop and maintain formal and effective partnerships with specialist mental health servicesWork to consumers individualised plans and track progress in line with funding requirementsAdministration and record keepingSkills & Experience Certificate in Mental Health or relevant Health Services (preferred)Demonstrated experience working with people with mental illness or psycho-social disabilityPassionate about client outcomes and mental health advocacyHigh computer literacy and administration skillsAbility to work autonomouslyCurrent Provide First Aid CertificateCurrent valid driver's licence and reliable vehicleSuccessful candidates will be required to clear probity checks including National Criminal History Record Check.
