About the role

As our Care Coordinator, you will be responsible for :
Building strong connections with clients and their families
Assessing client needs and resources for health, welfare and other facilities and services
Conducting Client Care Assessments and Care Planning
Maintaining accurate records as per the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework
Supporting all clients to meet their goals
Supporting and providing feedback to Home Care Workers
Re-organising work priorities to meet the needs of a fast-paced environment
Must promote the values of the company and prepared to plan supports using a contemporary approach
Essential Criteria
Experienced in assessing and planning care needs
3 years Support Worker experience
Sound understanding of the Aged Care Quality Standards and NDI Practice Standards, Quality and Safeguarding Framework
Organisational, administrative and time management skillsAll candidates must have the right to work in Australia
