Key Responsibilities
* Conduct comprehensive mental health and risk assessments for adults in the community
* Develop, implement, and review individualized recovery and treatment plans
* Provide case management and evidence-based interventions in line with best practice
* Deliver outreach support to consumers in their homes or community settings
* Collaborate effectively within a multidisciplinary team of nurses, social workers, psychologists, and occupational therapists
* Engage with families, carers, and community agencies to support consumer recovery
* Maintain accurate documentation in accordance with clinical and legislative standards
About You
* Tertiary qualification in Mental Health Nursing, Occupational Therapy, Psychology, or Social Work
* AHPRA registration or relevant professional association membership
* RPN3 level
* Strong understanding of trauma-informed and recovery-oriented care
* Excellent communication and interpersonal skills
* Current driver's licence and own car
