As the Intake Clinician, you'll work closely within the Intake Team to support clients accessing a wide range of community health services. This includes initiating intake processes, creating episodes of care, scheduling appointments, and maintaining accurate client data and documentation.
About you:
Relevant qualifications as a Nurse, Allied Health Assistant, or Allied Health Professional
Experience in person-directed care within a community setting
Strong interpersonal and communication skills, both written and verbal
Excellent organisation and time management skills
A team-oriented approach with the ability to collaborate across disciplines
Proficiency in digital systems and general computer skills
Commitment to ongoing professional development
Advantageous/Highly Desirable:
Knowledge of Primary Care and Chronic Disease Management principles
Familiarity with Uniti (client management system)
Understanding of accreditation programs and related legislation
Current Working with Children Check (or willingness to obtain)
Current Police Check and NDIS Worker Screening Check (or willingness to obtain)
Valid Driver's Licence
Immunisation History
