Overview
We are seeking an Occupational Therapist to join Amplar Health Home Health – Allied Health in Bunbury, delivering high-quality, community-based care to clients supported through Support at Home programs. This is a permanent role with flexibility, offering 0.4 FTE through to full-time. The role involves managing a Support at Home caseload and delivering client-centred OT services in homes and community settings across Bunbury and surrounding areas.
Responsibilities
* Deliver client-centred occupational therapy services to Support at Home clients
* Complete home and environmental assessments, prescribe equipment and plan home modifications
* Support falls prevention, reablement, wellbeing and functional independence
* Develop care plans aligned to funding and service delivery guidelines
* Maintain high-quality clinical documentation, reports and progress notes
* Work collaboratively within a multidisciplinary allied health team
* Deliver face-to-face care, with telehealth used where clinically appropriate
About you / Qualifications
* Full AHPRA registration as an Occupational Therapist
* Experience in community, aged care or home-based settings (highly regarded)
* Strong time management skills and the ability to manage a flexible caseload
* A client-centred approach focused on wellbeing, independence and quality of life
* Confidence working as part of a multidisciplinary team
* Flexible working arrangements, with hours negotiable from 0.4 FTE to full-time
