HIP Care coordinator
Based in Gippsland, Latrobe Regional Health are looking for a HIP Care Coordinator to join our Health Independence Program on a full time or parttime basis.
Our HARP team manages individual clients and also works with our Community Rehabilitation Service to run group programmes to support people with chronic disease - pulmonary and heart health.
The HIP Care Coordinator is responsible for triaging inpatients including clients attending our emergency department to assist with case management in partnership with their GP and other supports to promote effective management of their care needs.
Responsibilities
* Parental leave cover
* Chronic disease case management (cardiac, respiratory, age related)
* Participate in rehabilitation programs
* Client assessment and management
* Monday - Friday, flexible start times

Qualifications
* Registered nurse qualification
* Experience in cardiac or pulmonary nursing
* Excellent interpersonal skills and the ability to work well as part of a team
