About The Role
We're offering a full-time, 12-month maximum-term position for an experienced, enthusiastic community care professional to provide independent, ongoing advice and guidance to our clients living across the Greater Shepparton, Moira, Wangaratta, Indigo and Benalla regions. The role is primarily based from home with a monthly requirement to attend our Abbotsford office - overnight accommodation & travel costs are funded.
In this role, you will:
* Review intake information, engage new clients, establish home care agreements, care plans, budgets and connect our clients with local services and community resources
* Conduct client visits practicing person centred care planning, provide independent advice to clients and their families - enabling personal preferences and independence at home
* Interact with Care Connect team members, stakeholders, partnered service providers representing the best interests of clients and Care Connect
* Conduct annual reviews for all clients - revise care plans, goals, service provisions to quality standards and budget expenditure and maximisation
* Ensure service provisions are conducted ethically, fairly and within the statutory legal and contractual requirements at all times
About You…
You enjoy engaging with people, value relationships, and confidently deliver person-centred care plans to a diverse client base. You are committed to treating others with respect and maintaining confidentiality. You have excellent interpersonal, communication, networking, and negotiation skills, with the ability to prioritise, delegate, and organise tasks effectively. You bring experience managing client budgets and a strong understanding of Support At Home packages, Aged Care Standards, community resources, and service networks for older Australians and their carers. What you'll bring:
* Previous Care Management experience in Aged Care in rural communities
* Diploma or degree in Health/Allied Health OR 6+ years' relevant experience
* Strong communication, advocacy and interpersonal skills
* Proven time management, prioritisation and budget management skills
* Knowledge of Aged Care sector changes
* Current Australian driver's licence and reliable vehicle
* Proficiency in Microsoft Office and willingness to learn internal database systems
To be considered for this role you must have full Australian working rights, current Police & Working With Children Checks - we can arrange police checks for successful candidates. Our process also includes psychometric assessment.
