ESP Support Services is an Albury/Wodonga based, NDIS registered provider, offering a range of quality individual and flexible support options for people with a disability. Our mission is to enable our participants to live as independently as possible, by strengthening their ability to achieve their goals through the provision of ethical, innovative, and compassionate supports.
As a Support Coordinator/Psychosocial Recovery Coach, you will be a passionate and dedicated individual, who will work with our participants and their families to assist them in understanding, implementing and coordinating their NDIS Plan. You will help to identify their goals and support them to access the services they need - ensuring the participant's choice, control and independence is at the heart of what you do.
This role is a maternity leave cover position part-time, starting three days per week, with potential to increase to full-time as the case load increases. This is a work-from-home role based in the Albury/Wodonga Region.
Your Responsibilities:
* Establish rapport while building strong working relationships with our participants.
* Assist participants and their families, carers and nominees to understand their rights and build their confidence and abilities to make informed decisions.
* Monitor the funding and services provided to the participants.
* Work with participants to create a schedule of services, which support their needs and goals.
* Ability to work independently while maintaining strong communication and time management skills.
* Regularly review and refine plans with participants and report outcomes to the NDIA through progress reports within the specified timeframes
* Coaching to increase recovery skills and build capacity, including motivation, strengths, emotional resilience, and decision-making.
* Writing comprehensive case notes that are objective, clear and concise
Required skills and qualifications:
* Essential - minimum of 12 months experience in a Support Coordination role.
* Qualifications in mental health (minimum of Certificate IV in Mental Health) or equivalent training (Psychology)
* Working with Children Check in both NSW and VIC
* National Police Clearance
* NDIS Worker Screening Check
* Current driver's licence with a reliable car and comprehensive insurance
