This position focuses on providing evidence-based diabetes management, patient education, and self-care strategies for individuals living with diabetes.
As a Clinical Nurse Diabetes Educator you will work collaboratively with multidisciplinary teams in a rural health setting.
Responsibilities
The Clinical Nurse Diabetes Educator role includes advanced clinical and problem-solving skills, expert planning, and coordination in the clinical management of patients/residents/clients with complex care needs.
You will:
Practice autonomously and provide leadership in clinical decision-making for community-based diabetes education.
Provide professional direction and support to clinicians in this specialty area.
Collaborate with team members to lead practice changes and clinical innovation.
Deliver an integrated multidisciplinary coordinated approach to improve health outcomes for patients with complex diabetes and co-morbidities.
Reduce the risk of long-term complications and hospitalisation through evidence-based strategies.
