About the role:
The Discharge Patient Flow Concierge Social Worker role plays an integral role as part of the multi-disciplinary team to provide social work assessment and intervention to current inpatients across MLHD sites, focusing on complex care planning and overcoming psycho-social barriers to discharge or transfers of care from any MLHD facility.
The Social Worker can provide support with accessing a wide array of external social and community services, especially those with a welfare focus. You will also build strong links with local transport agencies and Community Transport providers, as well as local funeral homes.
Service provision will predominately be via virtual platforms, in order to facilitate equitable access to the service for all non-base hospital facilities.
The Social Work position will operate 7 days a week and report via the Acute Network Lead Manager and will be supported professionally by the existing Social Work structures within MLHD.
This position includes weekend coverage. The successful candidate will have the opportunity to job share and exercise flexibility in selecting work hours that align with their personal lifestyle and commitments.

About You
Our ideal candidate will demonstrate:
* A tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
* Ability to effectively prioritise and organise own work/ caseload, including effective utilisation of virtual care to meet service demand.
* Excellence in written, verbal, interpersonal and negotiation skills with a high level of problem solving and service evaluation skills.
* A comprehensive, all-encompassing approach to organisational goals, strategies and operations.
