A rewarding opportunity awaits for a Community Nurse/Clinical Care Partner to join a highly respected not-for-profit organisation delivering care and support services across Busselton and surrounding areas. This is a permanent part-time role with the potential to transition into full-time, offering flexibility and purpose in a community-focused setting.
As a Community Nurse/Clinical Care Partner you will:
* Collaboratively support BaH clients approved for clinical service types under Support at Home
* Support admissions of SaH clients into BaH
* Tasks associated with Clinical Care Partner defined within Support at Home program
* Have experience working within Quality Standards in the Aged Care sectorUndertake clinical assessments of home care clients.
* Providing excellent care and service to clients in line with their personalised care plans.
* Monitoring chronic diseases and palliative conditions.
* Wound care, continence care, palliative care, medication management and management of chronic diseases.
* A team player you will work closely with our care partners to ensure quality service delivery.
* Achieving the optimum care for clients by being responsive to their changing needs.
* Liaising with and referring to other organisations as required to provide for the clients needs.
* Assist the clinical leads with other duties as required.
Essential:
* Current AHPRA registration.
* Availability to work (M-F) - (adhoc weekends where required). Minimum 3 days a week.
* Aged Care experience is an advantage.
* Excellent verbal and written communication skills.
* A genuine enthusiasm, care and compassion in your commitment to working with the elderly.
* A current "C/CA" class WA Driver's License is essential.
* A National Police Clearance is required.
* Influenza Vaccination required.
