The Position
The Internal Coordination Manager is a senior leadership position responsible for overseeing Team Leader/s and clinicians to deliver exceptional care outcomes, through care partner coordinators, within a coordinated (self-managed plus) care model. This role focuses on optimising internal care coordination operations, fostering a culture of excellence, client satisfaction and continuous improvement. By collaborating with other leadership team members, the Internal Coordination Manager ensures seamless service delivery while aligning team performance with organisational priorities.
This role is accountable for compliance with aged care legislation, team development, and driving performance metrics all while prioritising client satisfaction and operational efficiency.
What You'll Do
Lead and support Team Leaders and clinicians to deliver high-quality care outcomes within a coordinated self-managed care model.
Oversee and optimise internal care coordination operations to ensure smooth, effective service delivery.
Foster a culture of excellence, client satisfaction and continuous improvement across the coordination teams.
Collaborate with the broader leadership team to align performance, priorities and organisational goals.
Ensure compliance with aged care legislation, policies and quality standards.
Drive team performance, including workforce development, coaching and achievement of key operational metrics.
Champion client-centred service, balancing efficiency with exceptional care experiences.
How You'll Succeed
Improve operational processes, efficiency, and overall team performance.
Strengthen collaboration and communication with service partners and external stakeholders.
Provide effective professional development, coaching, and support to Team Leaders and clinicians.
Maintain consistent compliance with aged care legislation and regulatory requirements.
Achieve organisational objectives while modelling and reinforcing Trilogy Care's values.
Resolve escalations promptly, mitigate risks, embed learnings, and deliver positive service experiences to stakeholders.
What You'll Bring
Tertiary qualification in healthcare or a related field (preferred).
4+ years' leadership experience in human services, with a strong record of developing high-performing teams.
Experience in aged care or healthcare, particularly in care coordination roles.
Ability to build and manage effective partnerships with external service providers and stakeholders.
Excellent communication, coaching, and conflict-resolution skills.
Strong analytical, problem-solving, and data-informed decision-making capabilities.
Solid understanding of aged care legislation, funding models, and compliance requirements.
Proven ability to implement organisational strategy and lead continuous improvement initiatives.
