Our organization is seeking a Support Associate to assist clients in participating in activities and tasks. This includes employment, in-home support, and community access.
The successful candidate will provide direct care assistance to each client in accordance with their service plan and as directed by the Manager.
* Support clients to access activities and outings in the community.
* Provide direct care assistance to each client in accordance with their service plan and as directed by the Manager.
* Work in accordance with medication administration policies, work health and safety procedures, and client rights policies including privacy and confidentiality.
* Monitor the safety and well-being of clients and report any concerns to the Manager.
* Provide training and support to clients in response to individual needs.
* Ensure completion of documentation, including recording relevant information on support provided in client notes and communication books as required.

Key Skills:
* Proven experience in disability support or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* High level of empathy and understanding of client needs
