About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
* Deliver contemporary high level Social Work/Psychology/Occupational Therapy clinical services (including assessment, formulation, treatment, risk management and brief intervention) within a person centred, assertive outreach, recovery oriented and trauma informed framework to consumers as a member of a multidisciplinary team, in accordance with prescribed professional and ethical standards.
* Co-lead and direct Multi-Disciplinary Team Review Meetings, which includes the review and co-ordination of, referrals, acute care/short term management, onward referral, and discharge of consumers.
* Actively participate and contribute to the co-leadership model led by the Team Leader, to ensure effective delivery of clinical and operational requirements.
