Victoria Practice is a well-established, highly regarded teaching and training practice based in a modern, purpose-built health centre in the centre of Perth. We care for approximately 10,600 patients and pride ourselves on delivering high-quality, patient-centred care within a strong multidisciplinary team.
We are looking for a Practice Nurse to join our experienced and friendly nursing team. This is an excellent opportunity for a nurse with an interest in chronic disease management (CDM) to develop and utilise their skills in a progressive and well-supported environment.
The Role
The role will focus on chronic disease management, with scope to be involved in a wide range of general practice nursing duties, including:
* Chronic disease management (e.g. diabetes, asthma, COPD, hypertension)
* Cervical cytology
* Health promotion and lifestyle advice
* Supporting patients to optimise long-term health outcomes
We use Medlink for CDM management, and full training will be provided if required.

About You
* Registered Nurse with current NMC registration
* Experience in primary care or community nursing is desirable but not essential, this role would be also ideal for a new qualified Nurse looking for a career in General Practice.
* Interest or experience in chronic disease management
* Strong communication and teamworking skills
* A proactive, patient-centred approach
