We are seeking dedicated Disability & Mental Health Support Workers to provide in-home and community access support for a participant in the Mornington and Somerville area.

About the Role
As a support worker, you will:
* Provide Community Access & Social Support
Assist with outings, appointments, shopping, and recreational activities to promote engagement and inclusion.
* Daily Living Support
Assist with routine household and personal care tasks while ensuring comfort, dignity, and independence.
* Mental Health Support
Offer emotional support, build trust, and encourage wellbeing through active listening and mental health strategies.
* Follow Behaviour Support Plans
Implement positive behaviour support strategies as outlined by professionals to manage behaviours of concern.
* Encourage Participation & Growth
Support goal-setting and personal development, and promote involvement in hobbies, social events, and the community.

To be successful in this role, you will have:
* Experience in disability or mental health support
* Ability to follow behaviour support plans
* Strong communication and empathy skills
* Reliability, initiative, and a positive attitude

Mandatory Requirements:
* Current Australian Driver's Licence
* Access to a reliable vehicle
* Current First Aid & CPR Certificate
* NDIS Worker Screening Check
* National Police Check
* Working With Children Check (WWCC)
* Full working rights in Australia
