Latrobe Community Health Service is a not‐for‐profit health care provider dedicated to preventing health problems through information and awareness and intervening sooner with truly integrated healthcare. Originating in Gippsland, Victoria, the service has grown rapidly in recent years and offers general practice, nursing, allied health, dental, counselling and psychology, gambling support, alcohol and drug treatment, migrant and refugee settlement services, social connection, carer support services and National Disability Insurance Scheme (NDIS) partnership with the NDIA.
What you'll be doing in this role
When you join the NDIS Service at Latrobe Community Health Service, you will be working with people living with disability to develop NDIS plans that support living life to its full potential.
Your Work Will Include
* Supporting people with disabilities engage with community and mainstream services to connect with supports and increase awareness of the NDIS.
* Monitoring and reviewing participant plans, providing recommendations to the NDIA to ensure supports build capacity and align with individual needs.
* Delivering services across sites and LGAs, providing face‐to‐face and phone support while meeting key performance and productivity requirements.
What You'll Need
* Relevant qualifications in community services (or similar) and/or demonstrated lived experience of disability, bringing practical insight and understanding to support participants effectively.
* A current Victorian Drivers Licence.
* A current Working with Children Check (Employee) or VIT Registration.
* NDIS Workers Screening Check (can be obtained during application).
What We're Looking For
* Strong organisational and planning skills, with the ability to manage competing priorities effectively.
* Demonstrated proficiency in ICT systems and client record management, ensuring accurate, timely and compliant documentation of participant information.
* Proven ability to work to and maintain Key Performance Indicators (KPIs).
* High level of empathy with strong active listening skills.
* Effective verbal and written communication skills.
