Patient Flow Coordinator (Clinical Nurse) – Mental Health Inpatient Unit, Mackay Base Hospital (Permanent)
Role summary: The Clinical Nurse (Patient Flow Coordinator) provides advanced clinical skills to ensure patient‐focused evidence‐based nursing care, coordinates the clinical management of patient/client care, and provides leadership and support to other nursing staff within the Mental Health Inpatient Unit. The activities are predominantly clinical in nature, but also support Nurse Grade 7 or above in management activities within the Division of Mental Health and Alcohol and Other Drug Services.
Responsibilities
* Provide professional guidance, leadership and support to nursing, medical and other professionals in the principles of decision making.
* Utilise advanced clinical skills and professional decision making to deliver a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and coordinating patient care in a positive way.
