About the role
North and West Remote Health (NWRH) is currently seeking an adaptable Journey Coordinator within the Stepped Care service system to join our team.
The Journey Coordinator is a dedicated position within the Stepped Care service system that operates as an independent partner in a person’s care journey and may be a standalone service delivered within a Front Door service or delivered as an adjunct to another Stepped Care service offering. The coordinator will ensure that individuals are placed at the centre of their own health and wellbeing and experience improved care and mental health.
Your primary responsibility is to deliver high‑quality support and services by improving outcomes for Stepped Care participants. Build psycho‑social education support to enable access to self‑management approaches including digital mental health services and group programs, while facilitating the Stepped Care model to ensure the quality, continuity and coordination of care between service providers.
This role is offered on a part‑time basis in Mackay and has outreach travel.
It’s challenging but we make a difference.
NWRH is seeking dedicated individuals who are culturally sensitive and adaptable with the ability to work both independently and autonomously.
You will have high‑level communication and negotiation skills to establish and support respectful and trusting relationships with individuals, teams, NWRH staff and the community at large.
Our teams come from across Australia and include a wide range of ages, skills and specialisations so you’ll be working with the best! This variety enables us to collaborate in our engagement with clients and learn from one another’s viewpoints.
