Silverchain Group invites applications for a Registered Nurse role focused on community‑based specialist palliative care. This full‑time position covers north and slightly south of Perth Metro, with a base at Kingsley/Osborne Park .
Job Description
Our Community Specialist Palliative Care Service supports clients and their families with life‑limiting illnesses at home. The role involves quality‑of‑life care through managing complex symptoms and providing advanced assessments and management for clients with changing needs. The successful candidate will be part of an interdisciplinary team delivering palliative care within the community.
Responsibilities
* Provide high‑quality nursing care to clients in the home environment.
* Conduct comprehensive assessments and create individualized care plans.
* Manage complex symptoms and adapt care plans as client needs change.
* Participate in team meetings and multidisciplinary case discussions.
* Collect consumables and attend required meetings at the Kingsley/Osborne Park base.
* Cover over 7 days per week, including days, evenings, and occasional night shifts (with penalty rates).
* Engage in extensive orientation, education, and ongoing training programmes.
Qualifications
* Registered Nurse with a minimum of 2 years of post‑graduate nursing experience.
* Current registration with AHPRA, no notations.
* Current Working With Children Check.
* Strong interest and some experience in palliative care.
* Acute care experience is highly desirable.
* Excellent organisational, time‑management, compassion, and communication skills (verbal & written).
* Current full Australian driver's licence.

Working Conditions
