We are seeking an experienced and compassionate Care Manager to join our leadership team at our 110-bed residential aged care home in Forbes. This is a permanent full-time, Monday to Friday role, offering the opportunity to work collaboratively in a well-supported leadership structure alongside the Residential Manager and two other Care Managers.
About the Role
As a Care Manager, you will play a key leadership role in ensuring the delivery of high-quality, person-centred clinical care to residents. You will provide clinical oversight, guidance, and support to nursing and care teams while contributing to strong clinical governance, compliance, and continuous improvement outcomes.
You will be an active member of the Home Leadership Team, working closely with the Residential Manager to drive quality care, resident experience, and staff engagement.
Key Responsibilities
5. Provide clinical leadership and oversight to nursing and care staff to ensure safe, high-quality resident outcomes
6. Support and oversee care planning, clinical assessments, and AN-ACC processes
7. Ensure compliance with Aged Care Quality Standards, accreditation, and clinical governance requirements
8. Coach, mentor, and support Registered Nurses and care staff to build capability and confidence
9. Work collaboratively with the Residential Manager and fellow Care Managers to support operational and clinical priorities
10. Build strong relationships with residents, families, visiting health professionals, and internal stakeholders
11. Contribute to continuous improvement initiatives and quality audits
12. Support incident management, risk mitigation, and clinical documentation standards
About You
You are a clinically strong and people-focused leader who enjoys working in a collaborative leadership environment. You bring sound judgement, emotional intelligence, and a genuine commitment to resident-centred care.
You will have:
13. Current AHPRA registration as a Registered Nurse
14. Previous experience in a Care Manager, Clinical Nurse, or similar leadership role within residential aged care
15. Strong knowledge of aged care legislation, quality standards, and clinical governance
16. Experience with AN-ACC assessments and care planning
17. Well-developed coaching, communication, and conflict-resolution skills
18. Ability to work both independently and as part of a multidisciplinary leadership team
19. Strong computer literacy, including clinical systems and Microsoft Office
