About Our Mental Health Role

We are seeking a skilled Care Navigator to join our multidisciplinary mental health team. The successful candidate will work collaboratively with consumers to connect them with services and supports in the community, facilitating person-centred and integrated care.

The role involves delivering practical support through information sharing and education, identifying and addressing barriers to access, and facilitating warm referrals to ensure consumers are linked to the right service at the right time.

As a Care Navigator, you will work closely with other members of the team and external service providers to maximise care outcomes for consumers.


Key Responsibilities:

* Develop and implement comprehensive care plans that meet the unique needs of each consumer
* Build strong relationships with consumers, their families, and carers to provide tailored support and guidance
* Collaborate with healthcare professionals and community service providers to ensure seamless coordination of care
* Identify and address systemic barriers to access, advocating for change when necessary
* Stay up-to-date with relevant legislation, policies, and best practices in mental health care


Requirements:

* Tertiary qualification in a relevant discipline (e.g., social work, psychology, nursing)
* Experience working in mental health, community services, or social sciences
* Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems
* Ability to engage with diverse populations, including those from culturally and linguistically diverse backgrounds
* Current driver's license and willingness to travel for the role
