Today, we have grown to support hundreds of clients across a variety of difference services under the NDIS and deliver bespoke Home Care Packages (HCP) which are tailored to assist consumers in remaining independent in their own homes for longer under our Aged Care model. About the Role
Within NDIS services, support people with a disability to achieve individual goals and enhance the participant’s inclusion win the community by supporting progress through a variety or planned and person-centred activities and developing independence in skills sets associated with independence.
Within aged care services, provide personalised 1:1 Home Care support to consumers accessing our Aged Care Home Care services. Our consumers in these programs receive ongoing care, during which time you will support their reablement and improved quality of life.
Key Responsibilities
* Domestic assistance such as meal preparation and light cleaning
* Assisting in acheivement of goals and building of capacity
* Personal care duties
* Assisting with shopping
* Transportation to medical appointments
* Implement tasks and routines outlined by an approved medical professional
* Support consumers in attending appointments and social activities within the community
* Maintain and monitor a consumer’s health and behaviour changes on an ongoing basis
* Build rapport and collaborate effectively with consumers to establish positive relationships
Skills & Experience
* A current First Aid Certificate and CPR Certification
* An Australian Driver’s Licence and ownership of a registered vehicle with comprehensive car insurance
* Strong communication skills with the ability to build and maintain positive relationships
* Passionate about making a positive impact in our ageing community
* National Criminal History Record
* Blue card and NDIS Worker Screening Check
* Certificate III in Individual Support Disability or Aged Care or equivalent
