The Aged Care Assessment Team (ACAT or Single Assessment Service/SAS) and ANAC Service (Australian National Aged Care Classification Funding for RACF) is a multidisciplinary team providing assessment services to clients in both the community and the hospital setting.
Responsibilities
The purpose of this position is to 'shadow assess' the funding needs of all new and current residential aged care recipients using the new Australian National Aged Care Classification (AN-ACC) tool.
Training in conducting AN-ACC/RAC assessments will be provided. Experience in aged care would be an advantage.
The successful candidate must be willing to travel between sites across the Illawarra Shoalhaven Local Health District.
