Job Overview:
As a Care Manager, you'll lead with purpose - supporting your team and making a meaningful difference to the lives of our residents every day.
Key Responsibilities:
* Oversee daily clinical operations ensuring residents receive best care available.
* Support and lead clinical team including coaching, mentoring, staff allocations and delegation, performance management and education.
* Review clinical incidents, complaints and changes, providing solutions to reduce risks.
* Lead compliance with AN-ACC assessments.
* Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers.
* Foster strong team environment.
* Prepare and conduct clinical governance audits.
* Provide insights and innovative solutions for continuous clinical improvement.
* Oversee medication management in accordance with resident needs and legislative requirements.
* Liaise with allied health, medical practitioners and other services to provide optimal resident care.
Requirements:
To succeed in this role, you'll need:
* Bachelor of Nursing with current APHRA registration.
* Post graduate qualification in aged care (highly regarded).
* Strong effective leadership, communication and interpersonal skills.
* A person-centred care mindset.
* A passion for nurturing strong team environment.
* Ingrained passion and empathy for our residents.
* Proven track record in delivering quality clinical care within aged care or acute setting.
* Excellent clinical assessment and triaging skills as well as strong care planning capabilities.
* Proven and demonstrated experience in fostering reporting culture balanced with investigative skills.
* An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes.
