What we're looking for
* AHPRA registered Occupational Therapist with an accredited degree
* Experience as an OT in a community setting, including undertaking home modifications and equipment prescription
* A passion for client-centred practice and collaborative care
* Ability to work collaboratively within a multi-disciplinary team
* Strong communication, time management, and organisational skills
* A valid full Australian driver's licence
The role
* You'll undertake home visits to support our Aged Care clients — assess their motor skills and endurance and assist them to engage in activities they find meaningful, like self-care and leisure.
* Deliver client-centred care that empowers clients to live their best lives – enhancing independence, achieving personal goals, and thriving in the comfort of their own homes
* Conduct holistic client assessments and develop individualised care plans that support physical, cognitive, and emotional wellbeing
* Build strong, supportive relationships and communicate effectively with clients, families, care providers and health professionals to establish goals, provide support and education
* Occupational therapy input may occasionally be required outside of Aged Care, with Health or Department of Human Services clients
* Work autonomously while staying connected to a collaborative team
* Manage your time effectively to meet client and program needs
