Get paid to live in paradise and join the Renal Services team as a Telehealth Clinical Nurse. In this role you will play a pivotal role in reducing the burden of chronic disease by working closely with patients through intensive case management. You’ll help them build strong self‑management skills and stay engaged with their medication plans - ultimately improving their long‑term health and independence.
About us:
The CKD telehealth service works with over 60 recipient sites to manage kidney disease for geographically isolated patients. Education regarding kidney disease and medication management is provided to patients, family members, carers, and remote health staff. The service outcome monitoring and performance measures/agreed deliverables are to increase access to CKD services for geographically isolated and hard-to-reach client groups by using telehealth for patients to access specialist renal physicians and clinical pharmacy services.

* Provide effective and efficient management for the telehealth service.
* Provide leadership to effectively plan, manage and evaluate the telehealth service.
* Problem-solve and demonstrate sound clinical judgement.
About your opportunity:
In this position you will…..
* Undertake quality improvement and evidence-based practice activities.
* Engage health professionals and patients within Cairns & Hinterland, Cape York, Torres Strait and Northern Peninsula Health Service Districts.
* Identify and maintain databases pertinent to telehealth support.
