* Employment Status: Permanent Full Time
* Location: Springwood & Surrounding Areas
* Remuneration: $37.77 p/hr + 12% Super + Not for Profit Salary Packaging (Increases your take home pay) + Allowances & Travel Reimbursement (KM's) + Employee discounts
About The Role
As a Senior Personal Carer, you'll conduct initial client intake and screening assessments and develop and review individualised care plans with a client-centered approach while providing quality care based on clients' needs.
* Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
* Management of client information in CRM & CMS ensuring all details and case notes are kept up to date
* Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team as required.
* Spend time on the road completing scheduled visits, interacting one-on-one with clients within their homes, supporting daily activities
* Provide care in accordance with individualised Care Plans
* Support community-based recreational and social activities
* Transportation and assistance with shopping and appointments

Role Requirements
* Certificate III Aged Care, Individual Support or an equivalent qualification – Essential
* Current Australian Driver's Licence (Green Provisional minimum) – Essential
* Roadworthy & insured (Third Party Property minimum) vehicle to use for work – Essential
* Experience with technology incl. smartphones, tablets, laptops etc
* Confidence to satisfy the relevant probity checks required by legislation or policies
