Family Support Practitioner
Location: Shepparton
Hours: 0.6 - 0.8 EFT (Permanent Part-Time)
Classification: SCHADS Level 5
Start Date: ASAP
About the Role
We're seeking a Family Support Practitioner to join a compassionate, family-centred palliative care service in Shepparton. This role offers a meaningful opportunity to support children, young people, and their families throughout every stage of their palliative care journey - from diagnosis and treatment, to end-of-life care, bereavement and beyond.
In this role, you will:
* Provide counselling and therapeutic programs to children, young people, and their families
* Support families in building strong social and community connections
* Coordinate and guide volunteers assisting with family care
* Collaborate with internal teams to deliver holistic, well-coordinated support
What You'll Need
To be successful in this role, you will have:
* A Master of Social Work
* At least 6 months' experience working with children, young people, or families in a therapeutic or support setting
* Excellent communication, collaboration, and organisational skills
* A compassionate and strengths-based approach to practice
