Behaviour Support Practitioner / Social Worker - Melbourne and across Victoria
Job Status: Full Time
Salary Range: $77,500 – $85,000 + super per year

Responsibilitieswill include:
* Assisting NDIS participants, along with their families and/or carers, to identify their needs through assessment and person-centred planning.
* Evaluating, developing and delivering evidence-based support plans within a complex support space.
* Developing meaningful goals and practical strategies utilizing a positive behaviour support framework, and with a commitment to eliminate restrictive practices, in the home and community.
* Ensuring individual plans reflect our NDIS participants' goals, passions and strengths, as well as ensuring they get more out of life.
* Building the knowledge, capacity and strengths of those who support participants in their lives - family, carers, friends, etc.
* Acting as an advocate and role model for best practice.
* Working collaboratively within a multidisciplinary team.
Qualifications
About you
* Full Australian work rights
* Registration as a NDIS Registered Behaviour Practitioner at core/ proficient/ advanced or specialist level
* Degree in Psychology; Counselling or Social Work or related area
* Min of 12 months relevant experience
* Current driver's licence
* Police check and Working with Children check (or ability to obtain these before commencing work).
* Excellent written, verbal and interpersonal communication skills - strong report writing skills essential
* Valid Driver's license and access to reliable motor vehicle
* The ability to work independently
