About the Opportunity
This role provides care to Renal patients in SWSLHD. The Nurse Practitioner/Transitional Nurse Practitioner assists in the clinical management of patients with CKD in conjunction with the Nursing team, Allied Health Team, Medical team and referring Nephrologist. The position delivers care across renal services in inpatient, outpatient and community health settings.
Key Responsibilities
* Assess, manage and treat Renal patients autonomously and collaboratively.
* Perform comprehensive health assessments and collect relevant clinical data.
* Prescribe medications, initiate diagnostics and interpret results.
* Facilitate direct referral of patients to other health care professionals.
* Collaborate with the Director of Renal Medicine, Nephrologists, Renal Nurse Manager and other stakeholders to identify, conduct and promote best patient outcomes.
* Lead educational initiatives for staff, consumer groups and patients; develop strategies for staff education.
* Promote clinical excellence, research and professional leadership; contribute to quality health care by developing clinical practice standards and evaluating outcomes.
Qualifications & Experience
* Registered Nurse with current Authority to Practice (AHPRA).
* Transitional Nurse Practitioner: Minimum 5 years full‑time post‑registration experience, including at least 3 years in the specialty field; appropriate postgraduate qualification.
* Nurse Practitioner: Master's Degree meeting NMBA endorsement criteria.
* Demonstrated ability to undertake comprehensive health assessments and make evidence‑based clinical, diagnostic and prescribing decisions.
* Excellent communication, engagement with patients, carers and multidisciplinary team.
* Strong leadership and mentoring skills; experience in practice development, policy, research and quality improvement.
