Clinical Care Coordinator Job Opportunity
This is a pivotal leadership role in one of our Residential Aged Care Homes. As Clinical Care Coordinator, you will work closely with the Residential Care Manager to lead the clinical operations and ensure residents receive exceptional care.
Key Responsibilities
* Organising and facilitating case conferences to develop care plans for consumers and their representatives;
* Leading the clinical care team and providing expert case management alongside the Residential Care Manager;
* Monitoring and improving clinical outcomes through regular audits and quality initiatives;
* Supporting policy development and implementation for resident care; and
* Coaching and inspiring staff to deliver outstanding care.
Requirements
* A registered nurse with current AHPRA registration;
* Proven clinical and leadership experience in residential aged care;
* Strong skills in assessments, care planning, and change management;
* Solid understanding of aged care funding models;
* Commitment to best practice and continuous improvement.
