About the Role
As a social worker within the Heart and Lung Stream, you will provide comprehensive social work services to patients, clients, and their families. This will involve delivering clinical assessments, counseling, and complex discharge planning, as well as supporting patients, families, and carers. You will also work closely with multidisciplinary teams to ensure high-quality patient-centered care.

Required Skills and Qualifications
You must hold a degree in Social Work and be eligible for AASW membership. You should have demonstrated clinical experience or understanding of the role of social work in a tertiary hospital or health-related setting. Clinical skills and knowledge in psychosocial assessment, counseling, therapeutic interventions, case management, advocacy, and discharge planning are essential.
