Community Nursing Role
This role offers a rewarding opportunity to make a meaningful impact in the lives of individuals and families. As a Community Registered Nurse, you will be part of a multidisciplinary team providing holistic and flexible nursing services.
You will work closely with customers in their own homes, assessing their needs and implementing preventative based approaches to healthcare through clinical skill and knowledge. You will also maintain accurate and up-to-date customer records, ensuring that care plans, progress notes, customer reviews, outcome measures, incidents, and consent are all documented effectively.
This is a mobile role, offering flexibility and autonomy as you plan your own schedule. You will have the opportunity to work in a fast-paced environment, utilizing your skills and experience to provide high-quality patient care.
Required Skills and Qualifications:
* A tertiary qualification in Nursing and current registration with AHPRA - Australian Health Practitioner Regulation Agency
* An understanding of the impact of ageing and/or disability on the lives of individuals and family members in the community
* Demonstrated experience in the provision of best practice clinical care, including clinical assessment, care plan implementation, wound care and management, medication support, continence assessments and management, clinical monitoring, nursing advice and recommendations, and general nursing duties
* Demonstrated ability to work efficiently in an autonomous capacity
* Effective communication skills to promote collaboration within the multi-disciplinary team and other organisations
* Excellent time management and organisation skills
* High level of computer literacy and effective and efficient data management and data entry
* Valid Driver's License and willingness to use your own vehicle for work
* Current First Aid and CPR Certificate and National Police Check
