Eye Health Nurse Co-ordinator Role
This role plays a pivotal part in delivering optimal outcomes for patients with ophthalmic conditions. The aim is to enhance access, coordination, and management of eye health services, including provision of care in remote outreach clinics, particularly for Indigenous and Torres Strait Islander Communities.
* Provide clinical assessment, coordination, and case management of outpatients in collaboration with Ophthalmology Consultants.
* E nsure the safe delivery of high-quality patient-centered care by adhering to policy, hospital protocols, and procedures related to clinical practice and environmental safety.
* Develop, deliver, and evaluate high-quality education and training resources for nursing staff, providing direct clinical support and supervision.
* Initiate, coordinate, and evaluate continuous quality improvement projects to reduce clinical variation in patient care delivery.
* Liaise with the Nurse Unit Manager and other team members to ensure effective communication regarding planned and unplanned changes in patient and clinic management.
Key Responsibilities:
* Providing clinical support to Ophthalmology Registrars before and after ophthalmic surgery.
* Partnering with key stakeholders and agencies to coordinate eye health services for Indigenous communities in the Far West Local Health District.
